Please remember that my terms of use still apply to adoptions which means that no altering is allowed, and the matching logo that I send to you with your adoption must be correctly linked to

Please fill out the form completely, and if the adoption will be given as a gift, please fill in the recipients information as well so I can keep track of my adoptions new homes. Also, if the adoption is for a gift, please specify in the instructions box whether you want me to send the gift for you or you want to send it yourself.
