Hrun the Barbarian was fighting a couple of gnolls on a crumbling ledge high in the Caderack Mountains (pg 39-40)
A fight breaks out at the Drum prior to the fire starting. Numerous assassins,
thieves, merchants, traders and assorted individuals involved. Those
unconscious when the fire took hold presumably died and those already dead
just got crispy. The fire spreads consuming most of Ankh-Morpork.
A high
death toll is likely.
A few members of the Gambler's Guild experimented with worshipping the Lady and they all died of penury, murder, or just Death within the week (pg 214).
White plague in Pseudopolis (pg 219).Death was said to be very busy. That was the reason Scrofula gave Rincewind for his absence. So we can assume a large number died.

The Schism of the Turnwise Ones, a pizza related Jihad, resulted in the deaths of some 25,000 people. The result being that the faithful were allowed to add a small bayleaf to the pizza (pg 109)
Five generations ago one of Kelly's ancestors halted his band of nomadic cut-throats a few miles from Sto Lat. They proceeded to attack and take control of the city (pg 114).
The runners up in the Citys eternal and Byzantine gang warfare were drifting down the Ankh river with lead weights tied to their feet (pg 155-156).

at the Battle of Quirm (pg 20).
The Battle of Koom Valley (pgs 415, 417) between the Dwarves and the Trolls. The only known battle in history where both sides ambushed each other. Given the nature of the two sides there was probably a large loss of life.
The battle between the Dwarves and Trolls near Quarry Lane (pg 209).
Referred to as the quickest, nastiest and above all narrowest battle ever held
in the city. Again a high death toll is assumed.

Some peasants who wanted to stop being peasants revolted. And since the
nobles won they stopped being peasants really quickly (pg 45).
There is an ongoing inter-tribal scuffle between the Vasung & Bergunds.
The most recent scuffle
(pg 102) saw the combatants picked up by 7 valkyries
who had to make several trips (pg 104). The scuffles originated 100 years ago
when one side invaded the others village and there was a bit of a massacre.
The others then returned the favour and after that there was some residual
bad feelings. The tribes seem to have been fighting and killing each other on
and off for these 100 years. There would have been a significant loss of life.
Klatch is incipiently at war with the D'regs and Hersheba. Hersheba is at war with Klatch and the D'regs. The D'regs are at war with everyone including themselves. This is due to the D'regs being either Klatchian D'regs or Hershebian D'regs depending on what side of the border they're on at the time.
Famous composers hang on to life for a long time. Most of them are deaf as posts when DEATH comes calling (pg 153).
The D'regs attacked the Klatchian Foreign Legion (pg 295). A large number of the Legion were killed.
