To place an order for the Omega License plates, see brother Philip White for your application. In order to purchase your plates you must be financial at the international, district and chapter level. You will be required to complete an Omega Psi Phi Emblem Use Authorization Statement along with $25.00 that goes toward scholarship. The next step is completion of a Form 1716 from the Missouri Department of Revenue along with $15.00. After retrieving your application from brother White, you can order your plates at any license plate office of the Missouri Department Of Revenue.
