1. We must repent of our sins.
2. We must place our faith in Jesus.   The Bible says "For by grace are ye saved through faith; and that not of yourselves: it is the gift of God."   Ephesians 2:8
        Faith is trusting in Jesus.
3. We must surrender to Jesus as Lord.

The Bible says "For whosoever shall call upon the name of the Lord shall be saved." Romans 10:13

Dear Lord, I know that I have sinned and need forgiveness. I now repent of my sin. I receive you into my life by faith. And commit my life to you.

Don't think, "When I get to the hospital, I will make this decision." Trust me, when I had the stroke, my family sent me to the hospital in an ambulance and I remember very little.
