When first posted as such in January 1964, SR 291 started at Jct US 2/US 195/ US 395 at the corner of Francis and North Division in Spokane and headed west on Francis, then NW to the Spokane-Stevens county line downstream from Nine Mile Falls. In 1983, SR 291 was extended via Tumtum to Jct SR 231 near Long Lake Dam. Today, SR 291 is 33.09 miles long.
Before 1964, SR 291 had been SSH 3S.
