Came before me Simeon Ashley, Clerk of the County Court, in and for the county and state aforesaid, the same being a Court of record, Jesse Fultz, whom I certify respectable and is entitled to full faith and credit upon his oath. And made oath in due form of law, that he was well acquainted with Robert S. Myers, who was a Private in Co. E, 1st Alabama Vidette Cavalry, In the service of the United States in the late war of rebellion with the Southern states. And that my age is 48 years. And that I was a member of the aforesaid Company. And know as a fact that on or about the 1st day of April A.D. 1864, the said Robert S. Myers, while in the service as aforesaid, and at the town of Stevenson in the State of Alabama, was diseased by exposure while in said service, as what I was informed by his Physician Doctor Thomas P. Stevenson “Hart Disease” contracted as aforesaid. And that on the 7th day of March, A.D. 1878, the said Robert S. Myers died with the aforesaid disease, as before stated. And affiant further states that said Physician as aforesaid was called to see the said Robert S. on his return hom after being discharged about the 15th day of July A.D. 1864. And that the said Robert S. lingered with said disease until his death as aforesaid, being unable to perform any labor from the time he arrived at home as aforesaid up to his death. And that I have no interest in the prosecution of this claim, and that my Post Office address is Pelham, Grundy County, Tennessee.
And I as Clerk of said Court as aforesaid certify that the foregoing affidavit was by me read over and explained by me to Jesse Fultz before he made his mark and swore to the same.
