Ordering Information

On a plain sheet of paper, print clearly or type the following information:

  • Your Name
  • Your Address
  • The name and order # of the item you want.
  • Color and/or magnet choice?
  • How many do you want of each item?
  • The total dollar amount for your order.
  • And a telephone number or email address where we can reach you in case we have a question about your order.

    • Send your completed order form along with your check or money order made out to Anne Watkins to:
