Passports

You can obtain a passport by going to a local postoffice. A list of post offices in the San Fernando Valley are as Follows

  1. 1. Completed application form DSP-11. This form can be obtained at a post office or county clerk's office, or you may print the application form directly. If replacing a lost passport form DSP-64 must also be completed.
  2. 2. Two Professional Passport photographs, taken within the past 3 months.
  3. 3. Proof of U.S. Citizenship: a) certified birth certificate, issued by state or county health department or vital statistics bureau (not hospital certificates); b) naturalization certificate; c) certificate of citizenship; d) Consular report of birth abroad; or, e) previous U.S. passport (if applicable).
  4. 4. Take the above requirements to your local acceptance agent for Passport Applications (usually a post office or county clerk or court house). Pay the clerk the U.S. government fee, including rush fees and execution fees, ($95 for adults, $75 for children under 16 years. Be prepared to write two checks for the total amount, as the clerk will usually retain $15.00 execution fee). Please write your name and date of birth on the check. Ask the clerk to execute the documents and place them in a sealed envelope addressed to "U.S. Passport Agency". (You must not open this envelope again before shipping it).
  5. 5. Include two copies of a Letter "Attn: U.S Passport Agency" stating: "I, (applicant name), hereby authorize Travel Document Systems to pick up my passport, and to discuss the status of my application with the Passport Services staff." Click here to print this letter from your browser. One copy of this letter, with original signature in BLUE INK, should be placed inside your sealed envelope by the passport clerk and the other should be paper-clipped to the outside of that envelope.
  6. 6. Include proof of departure date (two copies) required (copy of itinerary or air tickets showing applicant is departing the U.S., or letter from employer stating immediate international travel is required). One copy of the itinerary should be placed inside your sealed envelope, by the passport clerk, and the other should be paper-clipped to the outside of that envelope.

    TDS Fees to Hand Carry Application: $95 per passport if issue is required same day, or less than four business days
    $60 per passport if issue is required in between 3 days and 10 days
    $40 per passport if issue is required between 10 - 30 days.

    Return Postage $15.50 for return Federal Express (weekday priority)
    $25.50 for return Federal Express (Saturday delivery)
    Please Include a check or money order payable to "TDS Inc." for TDS Service Fee per passport, plus Return Postage or Federal Express per package.

    URGENT APPLICATIONS SHOULD BE SHIPPED TO TDS USING ONLY THE FEDEX COMPANY Processing Information
    Your application will be hand carried to the Passport Agency in Washington DC. There, the Passport Agent will examine the application and if all documents are in order, assign a date the passport will be issued, according to departure date from the U.S.
    SAME-DAY issue applications require proof of departure within four business days of being submitted to the passport agency and MUST be shipped to TDS using only the FEDEX Company.
    Urgent applications can be issued as fast as four business days, less urgent applications are issued within seven to ten days.
    Passport applications submitted by mail to passport agencies routinely take 4 to 6 weeks to be processed. TDS expedites applications by hand carrying them to the U.S. Passport Agency, and following up on the progress. Please note that TDS is not in a position to be able to offer any guarantees of passports to be issued by a specific date. Issue is determined by the urgency of your departure, the acceptance of your appplication without information being required, and passport agency workloads, particularly during peak travel periods.
    A 48-page business travelers passport can be issued in lieu of the usual 24-page passport on request at no additional charge. Attach a typewritten note requesting a 48-page passport to the passport application.
