§ 5-28-209. Contents of report.

Reports shall include the following information:

     (1) The names and addresses of the next of kin or persons responsible for care, if known;

     (2) The victim's name, address, age, sex, and race;

     (3) The nature and extent of the injury, sexual abuse, neglect, or exploitation, including any evidence of previous injury, sexual abuse, or negligence to the person;

     (4) The names and addresses of the persons responsible for the injury, sexual abuse, neglect, or exploitation, if known;

     (5) Family composition;

     (6) The source of the report;

     (7) The person making the report;

     (8) Photographs, videos, and X rays with the identification of the photographer and date taken; and

     (9) Other information that the person making the report believes may be helpful in the furtherance of the purposes of this chapter.
