RESEARCH- need info!

Hey everyone...last year I did a study on Self Injury, where I invited those 18 and older to answer several questionnaires.

I am now writing up this research and was hoping to get a bit of information I forgot to ask in the original study. Therefore I am asking those individuals who participated in that study to answer the following questions- PLEASE NOTE: If you did not take those questionnaires, then please do not fill out the following. I am only looking for those who have already taken it.

So if you HAVE taken this study BEFORE, and you are 18 years or older, please continue.
