Dear James,

Several days ago I received from you by e-mail the copy of the letter that you directed to Mary Jo Thompson at the First Health office in Houston. I appreciate the time you have taken to list all of your concerns and issues in such a detailed manner. Let me assure you that we will carefully review this matter to be sure that all of your issues are addressed.

James, in order to be sure that we are addressing all of the issues that are of concern to you, I would like to reiterate my request from my December 28 letter to you. Please list each claim specifically that you are appealing and the particular issue related to that claim. In her November 5, 1998 letter to you, Mary Jo Thompson provided you with a spreadsheet of claims that were submitted on your behalf. I would suggest you use this spreadsheet as an easy tool to provide me with your issue(s) on the particular claims that you are appealing. This list should be provided to Mary Jo and I would ask that you provide me with a copy as well.

The sole intent of this request is to ensure that we have a correct understanding of all issues you would like to see addressed for each specific claim. Hopefully this will expedite the review process and allow us to provide you with a definitive response to all of your questions.
