(There is a $10:00 donation for the certificate.)

FOR A SUCCESSFUL CEREMONY YOU WILL NEED:

1. __Dr./Physician's approval

2. __Dr./Mohel to perform ceremony

3. __Place and time of perform ceremony

4. __Information form completed and turned in

5. __Child should be already named

6. __Child should have a Hebrew name

7. __Kosher Wine/2 Bottles (With enough glasses for guests)

8. __$10:00 Donation for Brit Milah Certificate

9. __$50.00 Donation for Brit Milah ceremony

10. __Parent(s) in attendance

11. __Parent(s)/guests dressed in appropriate attire

12. __Please call the office as soon as possible if there are any changes in

plans, no matter how small

13. __Please be minded that the circumcision procedure is serious medical

procedure, so please adhere to all the guidelines of after care given to you by your

Dr. or Mohel.
