Inside his cell in Ryker's Island Penitentiary, Wilson Fisk remains clam as Peter Parker threatens to kill him – over the phone – for what he did to Aunt May. Fisk explains to Peter that he does not die. He has people that do that for him and some who do that because of him. For him to die would be a violation of the laws of nature. Furious, Peter replies that he has broken just about every law there is in the last forty-eight hours and that he is not afraid of breaking more to get to him. He then ends the call.
Holding by the collar the man who shot the sniper that shot his aunt, Peter tells him that once he pulls off the webbing covering his face, he is going to tell him everything he knows about the man he just shot. He then proceeds to yank off the webbing. Immediately, the man begs for his life, claiming that he does not know a thing about his victim. Peter knows the man is lying and threatens to hurt him if he lies to him again. Terrified, the man finally gives up and tells Peter that he was hired by the Kingpin to take the sniper out because he knew something the Kingpin did not want anybody else to know. The man swears that it is all he knows. The terror in the man's eyes lets Peter know that he is telling the truth. Though Peter said that he would kill anyone directly involved in the shooting of his aunt, the man is not and is simply being used by the Kingpin, same as he uses everybody else. However, he still needs to send a message. Suddenly, without warning, Peter hears Mary Jane's footsteps coming toward them. Not wanting the shooter to know that MJ and Aunt May are at the hospital, Peter grabs hold of him and web-slings into the night.
At Ryker's Island, Fisk tells one of the prison guards named Charlie that he wants him to do something for him. Charlie is willing to help, as long as it is within the rules. Fisk replies that he is afraid that he cannot help him there, because they are going to go far, very far beyond the rules. With that said, Fisk starts to destroy his desk, revealing rolls of money hidden inside the legs. As he continues to demolish his desk and other possessions, Fisk tells Charlie that he made sure, upon being incarcerated, to have ample supplies of money around should it every be required. Fisk explains that in ancient times, it was not uncommon for a king to be kidnapped and held hostage until a fee worthy of the kind was paid. In his present environment, Fisk must be prepared for any eventuality, however unlikely, including the possibility of someone making just such a move. Having filled his bed sheet with hundreds of rolls of money, Fisk asks Charlie if he knows what he is holding in his hands and if he can guess what it is called. Charlie guesses that it is called a king's ransom, which Fisk corroborates. Fisk tells Charlie that since his attorneys are negotiating some kind of deal with the justice department, which will see him released from prison, he no longer needs all that money, so he is going to buy something with it. Handing over the bed sheet to Charlie, Fisk tells the flabbergasted guard that the second thing he wants him to do is share this newfound fortune with the guards on night shift to convince them to look the other way at some point later that evening. Charlie agrees with the proposal and proceeds to ask what the first thing is. Fisk tells Charlie that he wants him to bring him his clothes, which are stored in the holding lockers downstairs.
Hiding in the sewers, Peter has the shooter hanging upside down from the tunnel ceiling. He tells him that he should kill him, even if he was not directly involved in what happened, because he was an accessory after the fact, which qualifies him by technicality. However, Peter has decided not to bother with him because he wants to save himself for the man who gave the order, not the guy lower on the food chain. Hungry sewer rats swarming around him, the shooter (named Jim) yells for help. Peter goes on talking about the food chain and how predators feed on prey, which are define as something incapable of defending itself in proportion to the attack, just like his Aunt May against a man with a high-powered rifle. Rats now climbing all over him, Jim begs Peter to release him, promising to do anything. Grabbing hold of a garbage can lid, Peter proceeds to sever the webline hanging Jim from the ceiling. As Jim falls into the sewer filth, Peter tells him that there is something he can do for him. Peter wants Jim to tell everyone in the whole wide world that his family is off-limits. Nobody touches them for any reason, ever. Anyone who tries anything against them will become prey to his predator.
A short time later, Peter shows up at the hospital to visit his Aunt May. Peter asks Mary Jane if there have been any changes. MJ replies that there haven't been any. The machines are still keeping her alive but her body is shutting down. If they lose power, for even a moment, she will not survive. MJ then explains that she is paying cash for all the medical expenses using their savings and they are starting to run out of money to pay for everything. Peter replies that he will somehow get more money to pay the expenses but MJ explains that they are drawing attention to themselves by paying for medical care in cash upfront. It normally always goes through an insurance company. Sooner or later, someone is going to figure it out and they are going to figure out who she is and whom she is related to (i.e. him, Peter Parker – Spider-Man, fugitive), then the bad people are going to come. Peter realizes what she is saying and tells MJ that the only option is to go for a Hail Mary play. He asks her if she remembers him telling her about that time he saved his aunt's life by allowing doctors to give her a blood transfusion of his blood (way back in Amazing Spider-Man #10). MJ replies that she does remember; however, she points out that the radiation in his blood almost killed Aunt May, which forced him to go through some pretty radical steps to get a cure to her in time (fighting Doctor Octopus in Amazing Spider-Man #31-33). Peter comments that she is right; however, he thinks that perhaps the serum had a lasting effect, which may now allow her body to accept his blood. When they did the transfusion, all these years ago, they purified his blood, taking everything out but the red blood cells to avoid fluid overload. His white blood cells could have saved her from having the reaction but had been removed in the process. So if they are to do it again, they have to have a straight-up transfusion, with no doctors to purify things or get in the way. Realizing what Peter is saying, MJ asks what he needs her to do. Peter instructs MJ about the equipment they require and she breaks into a storage room to acquire it. She then comes back to the room and they perform the transfusion.
A short time later, Peter tells MJ that if the transfusion has any effects, they should see it quite soon. He then tells her that he should go, as he has places to go and people to see. MJ replies that he shouldn't be going anywhere, considering that he just give a blood transfusion. She suggests that he lie down for a while. Peter responds that he cannot lid down because he has to be somewhere. MJ tries to argue with him, just as someone knocks on the door. As MJ answers the door, Peter takes off via the window.
Back at Ryker's Island Penitentiary, inmates are surprised to be awakened in the middle of the night when the whole prison is suddenly lighted up and their cell doors are unlocked. Wilson Fisk, the Kingpin of Crime, is not the least surprised, as he planned all this. As his cell door opens up, he finds a box sitting on the ground on the other side of the door. He opens the box to find a tuxedo. As Fisk gets dressed, Spider-Man breaks into the penitentiary undetected and makes his way inside. On the last page of the issue, Spider-Man and the Kingpin come face-to-face.
