V. FACT DESCRIPTION:

Describe the exact nature of your complaint below and on additional sheets if necessary.  Be brief, but complete.  Include the name of the individual that you dealt with and dates.  If possible, recite facts in the order in which they occurred.

On May 08th, 2005, I was attacked and assaulted by jailer Jay Abell without provocation.  This jailer hit me, kicked me and repeatedly stabbed my left arm until it began bleeding, requiring me to received medical attention.  This assault occurred when another (female) jailer known as Diamond refused to notify the kitchen of a food shortage and then refused to feed me and others.  Sgt. Sanders was called to the scene after jailer Abell, Lewis, and another (male) jailer escorted me to the holdover cell on the 11th floor of the George Allen Jail.  The jailers, Abell and Diamond, didn't tell Sgt. Saunders of Abell's assault/use of excessive force until I called jailer Lunsford over to the holdover cell I was in and showed him my bloody arm.  Sgt. Sanders came and asked me how I had been injured and I told her .  She then told jailer Lunsford to take me to medical where I was seen by a nurse and my arm was bandaged and placed in a sling.  I later was taken to a room where a man identified himself as Captain Ledford, the jail's facility commander, said he had been called from home to come and investigate this assault.  He asked me what happened, I told him.  He also asked to take a picture of my arm in the sling, and I complied.  He told me that he would pass the facts on to the proper personnel for further investigation, but to date I have never heard anything else from him and/or the Dallas County Sheriff's Office.

I certify that the information which I have furnished the District Attorney in this complaint is true and correct to the best of my knowledge and belief, and is furnished for the sole purose of instituting a criminal prosecution where the investigation indicates criminal activity and not for the purpose of recovering personal property or any other thing of value.  I authorize the District Attorney to use the information given in any manner which he deems necessary and proper.
