At first glance, you'll see that the trainer is trying to communicate something to the elephant. If you take a closer look, notice that the bullhook (the hook on the end of the stick the trainer is carrying) is digging into the elephants truck so much that its moving a considerable amount of skin.
Typically, elephant trainers will go for the more delicate parts of the animal (ie: behind/between the legs, trunk,etc.)
