Illinois 158 is an east-west route running through the southern areas of the Metro-East.

Intersections from west to east:

IL3 near Columbia

IL163 at Millstadt

IL15 by Belleville

IL158 joins IL13 eastbound

Both routes, now multiplexed, intersect IL159 in Belleville

IL13 breaks off and heads southeast; at this point, IL177 starts and follows IL158 eastward

IL158 leaves IL177 to go east as IL158 turns north

IL158 joins IL161 west for a short distance

IL161 breaks off

IL158 ends at a cloverleaf interchange at I-64 where US50 comes from the north and joins I-64 west-- this is near O'Fallon.

It passes through these towns: Columbia, Millstadt, Belleville, Shiloh.
