You can view the orderform here. If you want a text orderform, which is quicker to load and easier to print, please click here. Regardless of which orderform you use, please make sure you fill in your details clearly and in pen. All fields are required, unless noted otherwise. As noted on the How It Works page, I do find it easier to be more accurate if I have a fairly recent photograph of the querent. If you are able to provide one, please send it along with your orderform. I can return it to you with your reading, and it will not be copied, shared or made public in any way. You do not need to send a photograph. It is completely optional, but I stress again that I can be more accurate with a visual of the querent. If you are asking a question, please write your question clearly on the lines provided. Do not tick any of the spread boxes unless you have read the descriptions of each spread, and are selecting a spread that I have noted is available for specific questions. If you are not asking a question, but instead want a general reading, then do not write anything on the lines provided for the question. Tick the box next to the spread that you want a general reading for. If you are using the text orderform, place a tick or cross on the line provided next to the spread. To chose a deck, just circle or underline the deck you want me to use. If you don't have a preference, or aren't sure, just circle or underline the Not Sure option, and I will chose the deck that I think is best suited for your question or your chosen spread. When you have filled in your orderform correctly, send it, along with your moneyorder (for Australian dollars only, and made out to Naomi Ainsworth) or cash (in Australian currency only) to the address provided on the bottom of the orderform.
