Purposes: To raise funds for Pilot Dogs, Inc. so as to provide a pilot dog for a visually impaired person in Northern Kentucky. To increase awareness about health care in dogs.
Schedule: 11:00 AM - 12:00 PM Check in at BOONE COUNTY ARBORETUM at CENTRAL PARK on Camp Ernst Road, Union, KY 12:00 PM - 2:00 PM Walk along designated trail in CENTRAL PARK 2:00 PM - 3:30 PM Refreshments and Prizes at BOONE COUNTY ARBORETUM at CENTRAL PARK
Rules and Regulations
1. Participants must return the completed Entry Form to the Check-in-point between 11:00 am & 12:00 pm on the day of the Walkathon and agree to the Rules and Regulations set forth below.
2. Participants must be at least 12 years of age unless accompanied by an adult. Those 12 -18 years of age must have the agreement below signed by a parent or guardian.
3. Each participant is required to use the sponsor sheet on this form to get others to sponsor him/her for each kilometer walked. The total walk will be 5 kilometers. For example, a $2.00 donation per kilometer X 5 kilometers=$10.00. Participants should collect the donation when sponsorship is made. All donations will be turned in between 11:00 am-12:00 pm at the Check-in- point on the day of the Walkathon.
4. Prizes will be awarded for among other things the most sponsors obtained and highest amount of donations received. Presentation of prizes will follow the Walkathon at CENTRAL PARK.
5. All participating dogs should be in good physical condition, leashed and under the control of its master at all times. No vicious, unmanageable or ill animal will be allowed to participate. THE DOG OWNER MUST BE ABLE TO SHOW PROOF OF RABIES VACCINATION.
6. Dog owners are requested to make use of designated pet relief areas along the route in CENTRAL PARK and be responsible for cleaning up after their pets.
7. The Dog Walkathon will take place rain or shine. In case of bad weather or because of the strenuousness of the 5 k walk, participants may shorten their route if needed. If unable to attend due to weather, contact jmalvt@aol.com.
8. Parking will be available in the parking lots of CENTRAL PARK that are just inside the entrance to the park. Signs will be posted directing participants to the appropriate lots and Walkathon Registration.
9. The sponsors of the Dog Walkathon reserve the right to disqualify any owner or animal from participation.
BOONE COUNTY PARKS IS NOT RESPONSIBLE FOR ANY DAMAGES INCURRED TO PET, OWNER, PARTICIPANTS OR PASSERS-BY.

NORTHERN KY DOG WALKATHON SPONSOR SHEET

DEAR SPONSOR: The Northern Kentucky Veterinary Medical and Technician Associations are sponsoring the Northern KY Dog Walkathon for the benefit of Pilot Dogs, Inc., a non-profit organization to train and furnish pilot dogs to guide the blind. Funds collected will be earmarked for a qualified Northern Kentucky person. Pilot Dogs, Inc. provides the pilot dog, four weeks room and board for the student, all equipment, and round trip transportation at absolutely no charge to the student. The cost to the organization to provide one pilot dog is about $8,000. Your sponsorship of the above Participant(s) will help make this possible for a visually impaired person in Northern Kentucky.
YOUR CONTRIBUTION: You can donate any amount to the Northern KY Dog Walkathon, and your contribution is tax deductible. You may donate an amount based on the Five Kilometer walk or a total amount. For example, a contribution of $4.00 per kilometer equals a total of $20.00 ($4.00/km. X 5km.= $20.00). Checks may be made payable to Pilot Dogs, Inc. with a memo "For Northern KY Dog Walkathon ". Please give your donation to the above Participant when you are contacted. If you are unable to attend the event then contact jmalvt@aol.com for further instructions.
