You do not need to fill out the request form. All you need to do is send me your Angel's FIRST NAME ONLY and I will add them to the list of Angel's as shown above in the example. The form only needs filling out if you are requesting a personalized memorial that requires that you send me the links, photos, message, and all personalization information. The "descriptions" of each of the personalized candles is listed on the order form. If you want an example of the other candles it is available upon request.
