When I am teaching, I keep a notebook for each day of the week, with a section of the notebook for each student. Using carbon paper and notebook paper that I buy inexpensively in August, I keep a record of what I assign to each student for the week. I also have some sheets that I fill out containing important information about the student like address, phone number, practice records, and things like that. I keep the notebook with me at each lesson. There I record books that I have loaned out, music I have purchased, and goals for the student. I have all of the relevant information on each student, so that if a parent asks a question at the beginning or end of the lesson, I can tell them if the lessons are paid up or if there is anything that the student should be working on.
