While he was gone, his fiancée, Shouka, became ill with the same plague. She called and called for him, but no message could be sent to Mitsukake. By the time he returned, she was dead. Mitsukake blammed himself because he thought Shouka should have been his first priority. After that, he quit treating humans and became a hermit. He remained that wat until he met Miaka.

When Mitsukake first met Miaka, he was a ragged hermit, refusing to heal humans. He had become so after losing a patient, his fiancée Shouka, but Miaka spoke to him and convinced him that his healing was a gift that should be shared with others.
Mitsukake is a quiet, serious man with the practicality and level-headedness of a doctor. Although not as demonstrative of affection as the other Suzaku seishi, he has a great deal of compassion for others. He can't bear to see anyone, human or animal, suffer and he takes personal responsibility for the lives of all those around him. When he can't ease the suffering, he sees it as a failure on his part. But he is not in the habit of falling all to pieces. It seems Mitsukake always knows just what to say to ease another's grief.
