If you have lost a child and would like to include a memorial for him/her on my Memorial Page, please e-mail me, and include the following (be sure to type "MEMORIAL" -- all capitals, without the quotes in the subject line):
1.)name of child (as you would like it to appear on my page)
2.)Cause of death (i.e. stillbirth, miscarriage, SIDS, accident, etc.)
3.)Birthdate and date of death
4.)Age at time of death
5.)Name(s) of Parent(s) (as you would like it to appear on my site)
6.)E-Mail address &/or website address (and title) if you would like it to be included on my site (make sure to include your e-mail address or website address in the body of your e-mail if you want it included).
