Nowy Swiat 35, NIP 526-03-12-147, 00-029 Warszawa.  (I am not sure what all those numbers are; they were on the receipt.)  I read that this place had good donuts, and indeed they did!  A slightly booze-infused donut with chocolate on top with whipped cream in the middle and a cherry on top!  DAMN good!  Here's a dated review from the Warsaw Voice and a list of Warsaw cafes ; this lists their Blikle's alleged hours and other spots serving ice cream.
