years recent RN experience in hospital (with emphasis on pediatrics),
rehabilitation, critical care or emergency room is required. Residency
in the area, knowledge of community resources, the ability to access
them, and computer literacy are required as well as good communication
(verbal and written) skills. Case Management Certification (CCM) and
psychiatric nursing experience preferred. Must also be available to
assist facility discharge planners with discharge plans and
coordination of care for complicated cases. Home visits may be
necessary to more fully assess patient needs. Although this
position is home based, you will be part of a team locally and
at the Project office. Occasional overnight travel to the central
office in Austin is required. It is necessary that you be a resident
