Guidelines
and Recognition for Five Star Membership Program
Guidelines:
- Members are responsible for having their own form completed. Each
line should include the name of the activity, date, and signature/initials of an officer.
- If you run out of lines please come to the Phi Theta Kappa office in Union Hall 053 to
receive supplemental pages.
- Turn in your Five-Star Form at least TWO WEEKS before graduation or
transfer so we can arrange the correct recognition. If you turn it in later, we may
not be able to accommodate you.
Recognition:
Awards will be presented at
the annual awards banquet along with your star pins. To receive graduation
materials, please turn in your form two
weeks before your graduation. If you do not meet the star
status for the graduation materials, you may order them directly from Phi Theta Kappa International.
Members who complete an Enhanced
Membership Form will receive the Phi Theta Kappa Cords for graduation.
