Scott Leithead, the director of FORM, has completed Baccalaureate degrees in Honours Music (Wilfred Laurier University) and in Education (University of Toronto). He has also completed Orff Music Pedagogy Level 1 (University of Alberta) and Intensive Voice Study with Phil Mattson (University of Western Michigan).

In addition to FORM, he also conducts several award winning youth choirs including Kokopelli, The Edmonton Youth Choir, the St.Alphonsus Junior High Choir, the Octagon Group (a choir for people with disabilities), as well as two other Columbian Jazz Choirs, 'Nuf Sed and Postdated. He has conducted high school, college and community choral workshops throughout western Canada and across southwestern Ontario. Most recently Scott was an adjudicator at the largest festival of vocal jazz music in Canada, The Ontario Vocal Festival, Choralfest Manitoba and also at Musicfest Canada '98 in Vancouver B.C. He also directed the 1998 Eastern Manitoba Honour Choir.

Currently he serves as a board member of the Alberta Choral Federation, International Society of Music Educators 2000 and is a member of the Canadian Choral Festival Committee. As a member of ISME2000, Scott traveled to South Africa in July 1998 to represent Canada at the ISME1998 Conference in Pretoria.

Scott was the recipient of The 1996 Edmonton Salute to Excellence Award for outstanding work with youth.
