The GE M-250R shown here is still in excellent condition overall, and appears to have had relatively little use! The luminaire was made in 1974, the possibly orginial Westinghouse lamp inside was made in 1975. I believe this was an ex PG&E streetlight, since it came with a 4 foot long arm typically used by PG&E on freestanding steel poles in newer neighborhoods. The luminaire is a 175 watt mercury, and has a 120x240 volt ballast inside. It even came with a yellow tag that shows the instructions on how to set it up for 120 volt or 240 volt operation. When I find that tag I will post a pic of it here, so stay tuned!
Interestingly, this W lamp made in 1975 has a full frame arc tube support, the older design. Not sure when they discontinued the full frame support in BT-28 sized lamps, but later ones had a simplifed arc tube support.
