Report the death of an Air Force retiree by calling the local base or Headquarters AFPC's hotline (877) 353-6807, Monday-Friday, 7 a.m. to 5 p.m. central time. Voice Mail is available after duty hours.
Pay Problems
Retirees with pay problems may contact the retired pay section at the nearest Air FOrce Base or contact the Defense Finance and Accounting Service - Cleveland Center: toll-free (800) 321-1080 or commercial (216) 522-5534; or write DFAS-CL/FR,P.O.Box 99191, Cleveland, OH 44199-1126. Replacing Lost Air Force Documents
Request lost documents from the National Personnel Records Center: NPRC/NCPMF-C, 9700 Page Avenue, St. Louis, MO 63132-5000. Include full name, retired grade, Social Security number, and an explanation of your requirements, (such as a copy of a DD214, a record showing awards and decorations). A small fee may be charged for copying documents. Dependents' medical records are forwarded to an NRPC depository at 111 Winnebago St., St. Louis, MO 63118-4126 two years after the sponsor retires.
