Requesting A Transcript

When requesting a transcript, all requests must be made in writing to the attention of the counseling office at Daniel Boone High School. Transcripts are sent at no charge.

Please include the following

• Full name (when you were an attendee of Daniel Boone High School)
• Date of birth
• Social Security Number
• Year of Graduation (or last year of attendance)
• Name and address of Institution/Business/Individual to sent transcript
• Daytime telephone
• Your signature
