On yes/no questions, use this pattern:

Remember when you use a form of do to make a question, the
verb is always in simple form.

On yes/no questions using a question word, be sure to put
the question word first.

In a sentence that uses have to, or have got to, use a form of do to make the question.
I have got to go to the doctor this morning.
When do you have to go to the doctor?
Sally has got to leave at six.
When (What time) does Sally have to leave?
Remember: Have is the simple form of the verb.
