SSH 15B was added to the state highway system in 1937. This route began at Jct PSH 15 (now US 2) in Monroe and headed south via Duvall and Tolt (now Carnation), ending at Jct PSH 2 (at the time US 10, now SR 202) just north of Fall City. When the current state highway numbering system was first posted in January 1964, SSH 15B became SR 203.
