When I first saw this sheet, I was unsure just what it was for. After reviewing the script, and checking off the different scene numbers, I have surmised that it's a breakdown sheet of additional dialogue recording (aka ADR).

This was evident, in that some of the scenes mentioned, are large scene blocks, where there are a number of persons talking at a time. Most likely, the additional dialogue was some of the last stuff recorded on the film, or it's possible that the roles were voiced some time after production had wrapped.
