How to cite programs

Someone asked me how to cite my programs. I wasn't sure, so I did a Google search on "cite program". I also looked at the Uniform Requirements for publishing in Medical Journals. Here are the websites I looked at: The various suggestions translate to something like this, I guess:

Weaver, B. MA2x2.EXE [Computer Program]. Hamilton (ON): October, 2001. Downloaded from www.angelfire.com/wv/bwhomedir/programs/MA2X2.zip on DD-MMM-YYYY. System requirements: IBM PC, XT, AT, PS/2, or compatible computer.

My programs do not typically have version numbers. However, the month and year of compilation is shown on the first screen (October, 2001 in the example shown above). Substitute the date you downloaded the program where my example says DD-MMM-YYYY (and remove the italics).
