Free Children's Immunization Clinic
Rules for 2000

1. Immunization Clinics are FREE.
2. Please be sure to bring ALL immunization records with you to the clinic.
3. Children under the age of 18 need to have a signed immunization card by
a parent or legal guardian before any immunizations will be given.
4. Any additional questions may be answered by contacting the above address or telephone number.
5. Varicella (chickenpox) vaccinations are done by appointment ONLY.
