INDOOR TANNING CHECK LIST
- Operator has been instructed and given safety rules for operation of the units
- There are certificates/credentials posted to document training has been completed.
- A medical history card is completed for each client prior to exposure to the UV radiation.
- The medical history card specifically requests information on medications currently being taken .
- There is a medication/food/soaps checklist
- The medical history card requests information on pre-existing health conditions that may be exacerbated or contraindicate exposure to Ultra Violet (UV) radiation
- The client is at least (18) years of age, or has parental informed consent to allow exposure.
- The client card includes a record of prior tanning experience at this and any other tanning facility, including tanning in the home.
- The client card provides for Skin Type determination.
- Potential clients who cannot achieve a tan – very light skinned, highly pigmented, and aged for examples, are counseled regarding the limited or no potential value that may be achieved.
- Clients are specifically asked “Do you normally tan easily, do you have a condition that might get worse following UV exposure (such as cold sores), and are you pregnant?”
- Clients are advised regarding the known risks to excessive exposure to UV radiation, including the potential to get skin cancer.
- Eye protection is provided and required during all tanning sessions.
- The machine exposure timer is operative, accurate, and cannot be re-set by the client.
- Exposure times are determined by using appropriate time tables for the lamps and unit used.
- Sessions at this facility include recording the exposure times and unit used as well as the date and time of the exposures.
- The units and/or cubicles have a Hazard Warning sign that meets Food Drug Administration standards posted in a conspicuous location easily seen by anyone who might use the unit.
- The lamps used in the unit are spectrally matched and appropriate for the unit.
- Lamps are replaced in accordance with the manufacturers recommendations, and disposed of in a manner that complies with Environmental Protection Administration and other environmental protection rules.
- Cleaning procedures are adequate to protect clients from infectious agents, and up-to-date cleaning records are available for review.
- Fresh, clean towels are furnished to clients for each session.
- There is a clean, unbroken acrylic barrier between the client and the UV lamps.
- Live animals that may be a source of potential salmonella infections are not used around clients who present themselves for a tanning session.
- An injury reporting procedure is established, with an appropriate form and contact information for the client who sustains an injury as a result of attending a tanning session at this site.
- Advertising complies with Federal Trade Commission rules.
- The facility is registered and licensed where required with the state and local government to conduct business, and to provide indoor tanning services.
- The facility participates in national indoor tanning associations that promote adequate training, training updates, and vendor guidance on safe use of indoor tanning units.
