INSTRUCTION SHEET FOR PETITION
SIGNERS: Only registered voters from the same county may sign the petition. Because Registrar of Voters can only verify signatures for their own counties, please use separate petitions for signers from different counties. Print your name as you are registered to vote and your address, then sign your signature underneath your printed name and print your city and ZIP code.
CIRCULATOR: As the person who circulates the petition, even if you only circulate it to yourself, you must sign the Declaration of Circulator at the bottom of the petition. Circulators must witness signatures, even their own. Print your name, again as you are registered to vote and the address where you are registered to vote. Then fill in the dates that you circulated the petition; if the signatures where gathered in only one day, then fill in the same date in each space. Please print the name of the city where you completing the Declaration of Circulator, sign your signature as you are registered to vote and date it. Your petition is now complete.

SPECIAL INSTRUCTIONS:
Right-click on the S-O-S Petition link below and Save Target As and save the download to your desktop for best access. After you download the petition to your desktop open up the pdf file and look for the Page or Thumbnail tab on the left and then click on the tab. Now click on the top page (1) and then click on File, Print. After the page prints then turn the page over and put it back in the printer. Now click on the other page (2) and then click File, Print. You should have one sheet of paper with both pages printed on the same document with both sides going the same way vertically.
