The Forms in this directory are designed to provide you with an opportunity to understand the complexity of operating a teen court program under strict guidelines.
Each of the forms you will see contain most of the information that is required on the forms, however, some modification of the forms has been purposely left out.
It is abundantly important that anyone attempting to use the forms understand that they do not contain information on why the forms are used and exactly for what purpose they serve.
That is why there is professional training on the use of forms and procedures, and explanations of liabilities to individuals and organizations that place themselves at risk without the proper use of forms or procedures, etc.
