Entertainment Agreement:

Entered on this date __________between (DJ) and "The Client" as identified below. Upon acceptance of this Agreement,
DJ agrees to perform at the time and place specified. DJ shall furnish his collection of music and sound system as
requested by the Client at the time and place specified.

TERMS:
To insure the performance begins on time, the Client is requested to (a) designate in advance an area at least 10 feet by 10
feet for the performance, (b) locate the nearest 110 volt AC outlet and check its operation, (c) select the most direct and
secure route from unloading area to the performance area, (d) arrange to have at least 1 (one) banquet size table of at
least 6 foot in length available for the DJ’s use and (e) provide an adequate allotment of time, (one hour prior and one
hour post event) for the purpose of set-up and tear-down of equipment. In the event these preparations are not made and
the performance is delayed, the DJ will not compensate.
In the event the DJ is unable to perform during the entire agreed on time period, either due to travel, equipment or unforeseen
difficulty, liability is limited to providing the client with (a) compensation performance time equal to the time lacking or
(b) a refund of $15.00 for each full 15 minute time period of the agreed time that the DJ did not perform. Except for
situations where the performance is incomplete due to the fault of the DJ, the Client will be expected to pay the total stated
rate from the agreed start time.
In order to prevent liability resulting from accidental injury to any individual attending this event, DJ reserves the right to
deny any access to recordings or any equipment owned or leased by the DJ. In the event the DJ
feels a hazardous situation exists, it will be the Client’s responsibility, upon notification, to remedy the situation to the DJ’s
satisfaction. If no action is taken, the DJ may, in extreme instances, delay the performance, without penalty, until the
situation is resolved. The DJ assumes no liability for injuries to Client or guests.
The DJ encourages the Client to make recommendations as to specific musical selections to be played during the
performance, but cannot be held responsible if these selections are not played due to lack of availability. Unless
otherwise specified by the Client, the remaining music will be selected on the basis of popularity, dance ability and
demographic appeal.
The direction and control over the program presented at this event rests solely with the DJ and the Client shall have no
authority to supervise or control the DJ.
A 50% retainer is required at the signing of this contract to secure the date of the performance. The balance of the stated rate
is due at the Start Time of the performance. In the event the Client wishes to extend the time period of this
event, the rate for the additional time period ($________ per hour) will be paid along with the stated rate of the event at
the beginning of each extended hour of service provided. The event cannot be extended more than two additional hours
unless the DJ agrees.
As stated above, the DJ represents at the time of signing this Agreement, by either he or his Agent, to be available to perform
at this event. However, if the DJ becomes physically or mentally unable to perform, he will then, to the best of his
ability, provide a replacement as is required. If the Client concludes such replacement is not according to standards as
expected, he/she may so inform the DJ, requesting selection of a different replacement and the DJ will consider such
requests and accommodate them to the extent possible. If the Client is still not satisfied with the replacement, the
Agreement may be canceled by the Client and/or DJ without penalty to either.
If this Agreement is canceled by the Client, except as in (7) above, anytime less than 60 days prior to the performance date,
the full stated rate is due immediately.
Commencement of the event is deemed to be an acceptance of all the terms by the Client.
