Agree to enter into a contract for child care services with Donna XXXXX for the care of our child(ren).
Hours of Care:
I agree to provide child care for your child(ren) during the following hours:

Pick-up:
You agree to pick-up your child(ren) on time. An additional fee of $5.00 per 15 min. will be charged if you are late and have not notified me. In the event of an emergency, you agree to notify me as soon as possible and let me know what time you will arrive. You will need to sign a form prior to your child's first day of care indicating any individuals who are authorized to pick up your child(ren). You will need to notify me prior to any day one of these individuals (other than yourself) will pick up your child(ren).
Fees:
The fee for care will be. Weekly: $_________ This fee does not include diapers, pull-ups, wipes, formula, food if the child does not eat table food. You agree to pay this fee in full on Friday; which will be for care for the next week.
Trial Period:
Children of new clients will be placed in my care on a two-week trial basis this allows all parties to get acquainted, and if there are any personality conflicts, disciplinary problems, etc., each party has the opportunity, at an early stage, to terminate day care. For all spots held longer than one(1) week in advance, you will agree to pay half the regular weekly fee for each week your child is not yet in attendance to hold your child's spot.
Sick Time:
In the interest of all children. In the event that I become sick and not able to care for your child(ren) I will call you as soon as possible. When your child becomes sick at daycare, I will call you to come and pick up your child. You must pick up within one(1) hour if not than I will call the next person on the list. You will agree to accept and trust my judgment of how sick your child has become while in my care.
You are responsible for selecting a back up provider to care for your child(ren) when needed.
There are several forms that I must have completed and in my possession before I can assume the responsibility of caring for your child(ren). NO EXCEPTIONS.
I require ALL parents to sign their child(ren) in and out daily, there will be a folder with your child(ren)'s name by the front door when you come in.
I agree to provide an atmosphere of warmth, caring and concern for all the children in my care. I will provide nutritious meals, snacks, and beverages for every child.
We have read the Parent's handbook and understand all of the requirements.
We further understand that Donna XXXXX has the right to terminate care if the guidelines laid out in the handbook are not followed.
