CONTRACT & PARENT HANDBOOK

I will not discriminate against any child on the basis of their sex, religion, national origin or disability.
 
I am required by law to report all cases of suspected abuse and neglect to the proper authorities.
 
**Your child's adjustment is important to both of us.  It is agreed that a two-week adjustment period will exist before arrangements for regular care become final.  After this trial period, I will contact you for a phone conference to reaffirm and discuss final arrangements.  **
 
HOURS: Monday-Friday  7am-5:30pm  (see rates below)
 
RATES: 24 Months-5 Years  $95/week
             Infants     $95/week
  2 Children   $180/week
 
All Children must have current enrollment forms on file.  (Contract & State Enrollment form.
 
All Children must have a current and complete immunization form on file.
 
All children must have a current Physical Examination on file.
 
All children must have a Emergency Medical Treatment form on file.
 
 
PAYMENT:  Payment, $_____, will be paid every Friday evening at pickup for the following week.  Payments will be excepted in the form of cash or check.  I offer a 3% Discount if you choose to pay for the entire month in advance.
 
There will be a $30.00 fee for any returned check and you will no longer be able to pay by check.
 
You may be asked to pay for special trip expenses.  
 
LATE FEES: Late fees begin accruing when parent(s) is more than 10 minutes late picking up their child.  Late fees will also be charged when said payment is not received on the date agreed upon.  A $5 late fee will be charged for every 15 minutes (or fraction of) that you are late picking up your child(ren).  A $5/day late fee will be charged for late payment.  All late charges are due on the following Monday.  Excessive lateness is not acceptable.
 
TERMINATION:  Please give me a 2 week notice when terminating this contract.  If 2 weeks notice is not given payment is still required.  If I feel it is necessary to terminate this contract I will also give you 2 weeks notice (time to find another provider).  Reasons for termination may be:
   Excessive lateness with payments
   Excessive lateness picking up child
   Bringing child before contracted hours                  
ATTENDANCE:  Enrollment is FULL TIME.  If your child attends only one day of daycare, payment for the entire week is still required.  If your child will be absent payment is still required to keep space available each business day of the week.
 
EARLY ARRIVAL:  Please do not arrive before your scheduled arrival time.  
 
LATE ARRIVAL:  Please call if you will be late bringing or picking up your child.  Late fees still apply for late pick-up.
 
PICK-UP: Only the persons listed on the emergency form will be able to pick up your child(ren).  NO EXCEPTIONS. Please call if you will be picking up your child early.  
 
 
INFANTS: Parents of infants should purchase diapers and wipes to be left at the daycare.  This also applies to parents of toddlers still in diapers or pull-ups.  Bottles should be made and sent everyday.  An extra can of formula should be kept at the daycare for emergencies.  I will inform you when I am running low on supplies.  If your infant is beginning solids please provide me with the foods that he/she is eating.
 
PLAYTIME: Toys are provided by your childcare provider.  Please do not bring toys from home.  This can cause jealousy & arguments between the children.  Age appropriate toys, games and books are available for the children to play with.  If your child needs a special toy to sleep, it will be put away until nap time and put away after nap. Please no toy guns or weapons.
 
Children learn a great deal from our daily play activities.  Playing with play dough, finger painting, drawing, singing, reading books, or pretending are typical activities that we might do.  I strongly believe that children learn by doing and try to provide activities that are appropriate for children of different ages and interests.  As these activities can be a bit messy, I encourage you to dress your child(ren) in comfortable, washable play clothes every day.
 
If a child purposely breaks a toy, parents will be held responsible and will be asked to replace it.
 
OUTDOOR ACTIVITIES:  I try to take the children outside every day for fresh air and exercise.  If you have special requests regarding outdoor play (such as using sunscreen, insect repellent, etc.) please let me know.
 
MEALTIMES: Beginning 7/1/99 I will be planning/serving nutritionally balanced, wholesome foods, meals, and snacks, at no extra charge (for children that eat exclusively table foods). A menu for the week will be available in advance.    Children are encouraged (but never forced) to eat a variety of foods.  2 Meals and 2 snacks are provided for the children.  Parents of infants should provide their food and formula (milk).  This is the schedule for meals:
   BREAKFAST: 8:00am-8:30am
   AM Juice: MIDMORNING
   LUNCH: 11:30am-12:15pm
    SNACK: 2:30pm
If your child will be late for one of the planned meals, please let them eat before they come.  No gum or candy is permitted in the daycare (except special holidays).
 
 
NAPS/REST TIME:  Children in my care usually take regular naps.  Infants nap as needed, and older children generally sleep in the afternoons after lunch.  Even if children do not go to sleep, I still insist that they lay down for a rest time and look at books quietly.  
 
EXTRA CLOTHES:  All children, regardless of age, need to have a complete change of clothes on hand at my house at all times.  Please write child's name inside.  These clothes will be used in case your child has an accident, spills something on themselves or get wet outside.  Also send an old tee shirt or dress shirt for your child to wear as a smock when he/she is doing an art project.(ALL CHILDREN)
 
GUIDANCE AND DISCIPLINE:  I try to create an environment for children that is supportive and respectful of their needs.  I use different methods of discipline for different situations.  When a child/children are arguing or out of control, I will insist on a "time out" or cooling of period.  When children are doing something that is not appropriate, I will redirect them to a more suitable activity.  I will not physically punish your child by spanking, slapping, or hitting.  My goal is to help each child develop a strong sense of self-discipline and self-esteem.
 
INJURIES: All minor injuries will be treated with a Band-Aid or ice pack and T.L.C.  Any other treatment necessary I will call you and discuss treatment which you advise.  I am trained in First Aid and CPR.
 
ILLNESSES: Please do not bring your child if he/she is sick. I can only care for children with mild cold like symptoms (CLEAR runny nose, SLIGHT cough, and no fever).
 
 If your child develops any of these symptoms while in my care, you will be required to pick him/her up immediately. You will have a one hour window to pick up your child.  Before returning to child care, your child must be free of ALL symptoms for 24 hours,
or be accompanied by a doctor's note.
This benefits your child and the other children. Your compliance is appreciated, and will be compensated with the assurance that all ill children will be kept home. Please keep in mind that well children are demanding and sick children even more so. Please be fair to your child, the other children who come to my child care and to myself and do not bring your child to me when they are ill, I am running a *WELL* childcare home.
 If I am unavailable due to illness, I will give you as much notice as possible. If my substitute is unavailable you will be responsible for finding alternate care.  
 
Medications will be given only with the parent's or doctor's direct written consent.  All medications will be in their original container, labeled with their name, include written instructions on the amount to be given and the time to be administered.  Please bring your own measuring cup, spoon or syringe.
 
HOLIDAYS - OFF DAYS:  I take a few days off per year and I observe several holidays (New Years Day, Memorial Day, 4th of July, Labor Day, Thanksgiving, the day after Thanksgiving, Christmas.) The day I am closed is the day the holiday is "observed" . Payment is still required for these days.  If I take a sick day or any other day off, your payment will be pro-rated.
 
 
 
VACATIONS - I will be closed the week between Christmas and New Years for my vacation.  You will not be require to pay for this week.
 
You are entitled to one week of vacation without paying for your child's space.  You must provide at least two weeks notice (in writing) and your child may not attend any day during the week of your vacation. You may not split of this time it must be taken all at once.
 
PARENTS “NIGHT-OUT” - On occasion, on a Friday night I will give you a chance to drop off your child and have some time for yourself.  All children need to be picked up by mid-night.  There will be no charge for this evening.  This is my way of saying Thank you and giving you some much-needed time to yourself.  I will not let a child leave with a parent that appears intoxicated.  
 
EMERGENCIES:  In the event of a medical emergency, fire, or natural disaster.  I will take immediate steps to ensure your child's safety and will contact you as quickly as possible.  Please make sure that you keep me informed regarding your whereabouts.

I value the importance of teaching my own children basic Christian principles; love, joy, peace, patience, kindness, goodness, faithfulness, gentleness and self control. understanding that I am opening my home to your children, these values will be shared with them as well through stories, music, videos, etc.
 
I enjoy and take pride in my profession.  Parents sometimes forget that my services are much like any other business because I work in my home.  If you fail to pick up your child on time, I have less time with my family.  If you fail to pay your fees, I may find it impossible to pay my own bills on time.  If your family experiences an unforeseen difficulty, I am very willing to work with you.  However, I do ask that you be respectful of my needs so that I can continue to offer your child quality care.
 
 
 
I/WE HAVE READ AND AGREED TO ENROLL MY/OUR CHILD IN MS. CATHIE'S HOUSE FAMILY CHILDCARE. I/WE HAVE AGREED TO COMPLY WITH ALL RULES AND RESPONSIBILITIES STATED HEREIN. I/WE UNDERSTAND THAT MS. CATHIE'S FAMILY CHILDCARE RESERVES THE RIGHT TO MAKE ADDITIONS OR DELETIONS TO THIS CONTRACT WHEN PROVIDED A TWO-WEEK NOTICE. I/WE AGREE THAT IN THE EVENT OF NON-PAYMENT OF ANY FEES, MS. CATHIE'S FAMILY CHILDCARE WILL ATTEMPT TO COLLECT THE DEBT BY ANY MEANS AVAILABLE INCLUDING, IF NECESSARY, SMALL CLAIMS COURT. AT WHICH TIME I/WE WILL BE HELD LIABLE FOR ANY AND ALL ASSOCIATED FEES (INCLUDING LATE FEES). I/WE WILL INITIAL ANY MODIFICATION WHEN NOTIFIED. I/WE UNDERSTAND AND AGREE THAT THESE MODIFICATIONS WILL BE AS BINDING AS THIS CONTRACT.
