6. The expiration date of the Credit Card, i.e. 06/01 (four digits).

This is the business name underwhich the transaction will take place.
Yes, it is still us at Boyds Bearstones Secondary Marketplace.
(Your Credit Card will reflect this name as well when you receive your regular card billing.)
You will have the option of voice or fax.
