You will need to use Adobe Acrobat Reader
Version 4.0 (or above) to fully utilize the "data entry" features of these
forms. You can obtain a free copy of Acrobat Reader at
www.adobe.com/prodindex/acrobat/readstep.html.
Download these forms to a directory of your
choice on your computer's hard drive. If Adobe Acrobat Reader is installed
on your system then just double-click on the downloaded file in Windows
Explorer. This will launch the Adobe Acrobat Reader.
These forms have been designed so that you
can type information directly into the appropriate data fields and then print the forms on your computer's printer.
The forms' data entry fields may be selected by clicking
with a mouse in the appropriate field on the form. You can move between
the fields by pressing the tab button on the keyboard to move forward or
the shift + tab buttons to move backwards. When printing you should make
sure that "annotations" in the print dialog box is not checked. If you have the full version of
Adobe Acrobat, you can also save the form. Choose
File (on the Acrobat Toolbar) and Save As if you wish to save the
completed form.
Click on the
red Squareat the upper left
corner of the opened form to reset the form, i.e. clear data that has been
entered.
Click on the yellow question mark on the opened form to get
instructions on using the form. Click on the question mark or Window
button (top left) to close the help
window. |
