3. Proposed site or location (with accompanying vicinity or location map)
II. FOR CORPORATE OR PARTNERSHIP APPLICANTS
1. Items in I (1 to 3)
2. Articles of Incorporation
3. Board Resolution authorizing President/Manager to sign application and authorizing him to enter into Franchising Agreement and authorizing him to sign in behalf of the corporation
4. Certified True Copy of Securities and Exchange Commission's Registration of Corporation/Partnership
5. In case of partnership, resolution by the partners authorizing the Manger to sign in behalf of the partnership
6. Audited Financial Statement of the Corporatin/Partnership
7. Photocopy of Articles of Partneship
8. Photocopy of Community Tax Certificate of the Corporation/Partnership
