The forms below need to be printed and filled out and brought to the parents meeting on Sunday 12/4/08 (at 6pm at theYMCA Youth and Family Center). If you have difficulty with a section, bring it to the meeting and someone will help you.

*Please register online with USAV (Travel Teams only) and bring a printed copy of your confirmation page with you to the parents meeting (they do not allow us to submit registration forms as a club, they need to be done individually over the web). This can be done at www.usavfl.org . It requires online payment of USAV fees of around $45.
