*Applicable to full-time students only or 2003 BS Chemistry
graduates. The secretariat requires a copy of the proof of
enrollment.
The
registration fee entitles the participant to morning and afternoon
snacks, lunch on 21-23 May 2003, and a fellowship dinner. However,
only pre-registered participants can be assured of the Congress Kit
and Registration materials.
Full payment of registration fees with corresponding
official receipt is confirmation of participation in the Congress.
Payment may be done by cash or check addressed to Integrated
Chemists of the Philippines (ICP)-19th PCC or by depositing to
Bank of Philippine Islands (Loyola Katipunan Branch) Account
No.3081-0855-51. A photocopy/duplicate copy of the deposit slip
must be submitted with the registration form by mail or fax to this

TRANSFER/CANCELLATION OF REGISTRATION
Cancellation of registration may be done in writing before 30 April
2003. A 50% refund of the registration fee will be given to all
authorized cancellations. A letter of authorization, duly signed by
persons concerned must accompany transfer of registration.
