US12 enters the state of Illinois from Wisconsin near Richmond, Illinois, in northeastern McHenry County

IL173 at Richmond

IL31 at its northern end

IL59 joins US12 south near Fox Lake

IL134 at its western end

IL120 at Volo

IL176 at Wauconda

IL59 leaves toward the south at Wauconda (IL59 traffic has no access to/from the east on US12)

IL22 at Lake Zurich

IL53 joins from the north near Palatine

Shortly after that, US12 crosses IL68 as IL53 turns east onto IL68

Then US12 passes under an expressway extending from I-290 north and carrying IL53 (access to/from the south only)

IL83 at Prospect Heights

IL58 at Des Plaines

US12 joins US45 for what will be a long multiplex, still at Des Plaines

US14 at Des Plaines

IL72 at Rosemont

US12/45 then has an interchange with I-190, providing access to I-90, I-294, and O'Hare Airport

IL19 at Schiller Park

IL64 near Northlake

US20 joins from the west, forming a "tripleplex" of Federal Routes

US12 passes through these towns between Wisconsin and Bellwood: Richmond, Pistakee, Fox Lake, Volo, Wauconda, Lake Zurich, Kildeer, Palatine, Arlington Heights, Prospect Heights, Mount Prospect, Des Plaines, Rosemont, Schiller Park, Franklin Park, Northlake, and (of course) Bellwood.
